AQH representative, Motivational Counselling nurse from Fushe Kosova, the Coordinator from Center for Continuing Nursing Education in Ministry of Health, and Director of Directorate of Health in Gracanica Municipality have attended the 19th International Conference on Integrated Care that took place in San Sebastian, Basque, Spain from 1-3 April.
The conference brought together researchers, clinicians and managers from around the world who are engaged in the design and delivery of integrated health and social care.
The Municipality of Fushe Kosova model has been presented in the Conference as a pilot project of integrated care model that aims to improve the quality of care provided for patients over the age of 65yrs that have been diagnosed with Type 2 Diabetes, through better coordination of services provided by the health and social sectors.
