Workshop with international expert and core WG on Geriatric Assessment (GA) for integrated care was held. During this workshop the principles of GA and different assessment tools were introduced. The WG participants agreed on the process and modality for implementing geriatric assessment. Furthermore, it was agreed which tools to be used for implementation of Geriatric Assessment to support integrated care planning across sectors. The Geriatric Assessment tool was drafted by the international consultant whereas the WG participants modified the social aspect of the tool for the Kosovo context.
