Since we completed the Rapid Assessment in June 2017 the MFMC directors from 4 service package municipalities have been working with AQH project to review the findings of Rapid Assessment and identify areas for improvement. Each Municipality has developed their own Action Plans which will be used to guide the implementation of Service Packages for Diabetes and Hypertension in their municipalities.
